When the engine is not running, the pressure inside the intake manifold is the same as the pressure outside. When the engine starts, a vacuum is created inside the manifold due to the suction action of the piston and the restriction created by the throttle plate. With the engine running at full speed, the intake vacuum drops to almost zero and the pressure inside the intake manifold again approaches the external pressure.
